Sammi Jo Rae Midcap Koch, 38, of Bolivar died unexpectedly Thursday afternoon, June 18, 2026, in Altercare Navarre.

Sammi Jo was born in Canton on February 10, 1988, to the late Dale Midcap and Joni Leinhard Koch who survives. Also surviving are Sammi’s brother Scott (Jessica) Midcap of Brewster, stepsiblings: Ryan Koch, Marcia Koch, and Laura Koch; nephews and niece: Nathan, Bronson (Jalyn), Jermaine Jr., Stanley, and Rozlyn; maternal grandmother Nancy Lienhard, and many aunts, uncles, and cousins. Along with her father, Sammi was preceded in death by her stepfather Richard Koch, and maternal grandfather John Verne Lienhard.

Sammi enjoyed exploring new hobbies, especially arts and crafts. She also loved music, caring for animals, collecting hippos, and spending time with her family and friends.

A memorial service for Sammi Jo will be held on Monday, June 29, 2026, beginning at 6:00 PM at Koch Funeral Home of Gnadenhutten, 244 N. Walnut Street, 44629. Pastor David Koch will officiate.
